Attention All Athletes: Interscholastic Sports Physicals are Mandatory For Participation

District 25 policy requires that all students who wish to participate in extra-curricular sports activities have a physical examination, signed by a physician licensed to practice medicine in all its branches, approving the child’s participation. Proof of a physical must be on file in the school office one day prior to the start of tryouts for any sport. A student, who does not meet this requirement by the due date noted above, will be ineligible to tryout for or participate in the activity.

Sports physicals may be turned in at the Administration Center during the summer.

Sports physicals are valid for 395 days from the date the physical is performed. 

Any student who is not allowed to participate in interscholastic activities as the result of a physician's directive may not return to play until they have received written clearance from a physician (i.e. concussion, sports related injuries, etc).

NOTE:  Physicals are required for participation in all sports
