Parents and legal guardians of students new to District 25 this year should enroll their children at the Administration Center, 28W250 St. Charles Road, West Chicago as soon as possible to facilitate placement plans. A birth certificate and proof of residency for each child to be registered must be provided.

State law requires that students attend school in the district in which they reside. At the time of registration, parents/guardians new to the district must complete a Proof of Residency form as well as provide the following documents.

 Category I (one of the following documents showing proper address): 

  • Real estate tax bill
  • Signed lease
  • Mortgage papers
  • An Agreement of Sale
  • Affidavit from local resident attesting registrant is living with the owner at no cost

or, Category II (three of the following documents showing proper address: 

  • Drivers license
  • Voter registration
  • Auto registration
  • Home/apartment insurance papers
  • Gas or electric bill (telephone bill not acceptable)
  • Library card showing current address
  • Credit card bill
  • Public aid card

If a student is enrolled with misrepresented information and is found not to be a resident of the district, they will be dropped immediately from district attendance rolls. Non-resident students may attend district schools only if approved in advance by the superintendent and by prepaying the non-resident tuition fee of 110 percent of the district’s student per capita cost. This rate is set by the State School Code.

 Complete Your Child’s Immunizations

 In compliance with the Illinois School Code, Benjamin District 25 requires that all students entering kindergarten and sixth grade have a physical examination, performed by a physician licensed to practice medicine in all its branches, within one year of the start of school.  Proof of required immunizations must be recorded on the State of Illinois Certificate of Child Health form.

 Students who do not have required immunizations completed at the time must have a schedule for immunization completion and a signed physician statement of medical reasons for the delay. These documents are to be presented at the time of registration. Failure to present these documents will result in the student being excluded from school.

 In addition, all students entering Benjamin District 25 from a school not located in the State of Illinois must present proof of a physical examination performed within one year of entry and a record of all required immunizations recorded on the State of Illinois form, regardless of grade.

 PRESCHOOL:

  1. Diphtheria, Tetanus, Whooping Cough (DPT) - 4 doses

  2. Polio Vaccine – 3 doses

  3. Measles, Mumps and Rubella (MMR) – 1 dose after 12 months of age

  4. Hepatitis B – 3 doses

  5. HIB – number of doses depends on age given and type of vaccine used

  6. Tuberculin test – recommended, but not required.

KINDERGARTEN:

  1. Varicella – 1 dose

  2. DPT – 5 doses with the fifth dose given after the 4th birthday

  3. Polio Vaccine – 4 doses with the 4th dose given after the 4th birthday

  4. MMR – first dose after one year of age and a booster prior to kindergarten entrance. If the first dose was given before the first birthday, it does not count and two additional doses need to be given. (2 doses total)

  5. Hepatitis B – 3 doses

  6. Tuberculin test – recommended, but not required

 

 FIFTH GRADE

  1.  Hepatitis B – 3 doses with the first two doses one month apart while the third dose is administered four to six months after the first dose.

  2. Tuberculin test – recommended, but not required

Attention All Athletes: Interscholastic Sports Physicals are Mandatory For Participation 

District 25 policy requires that all students who wish to participate in extra-curricular sports activities have a physical examination, signed by a physician licensed to practice medicine in all its branches, approving the child’s participation. Proof of a physical must be on file in the school office one day prior to the start of tryouts for any sport. A student, who does not meet this requirement by the due date noted above, will be ineligible to tryout for or participate in the activity. 

Students trying out for soccer or volleyball in August must turn in their physicals before the 1st day of tryouts. Sports physicals may be turned in at the Administration Center during the summer. 

Sports physicals are valid for one year from the date the physical is performed.
