Vision and Hearing Schedule
Listed below is a chart of the grade level’s that will be screened for vision and hearing in accordance to the Illinois Public Health Child Vision and Hearing Act. Please note grade four vision testing is not mandated but recommended. District 25 does pursue vision screening of our fourth grade students. If a parent or guardian objects on constitutional grounds, and submits a written objectives to the school, the screenings will not be required.

In addition, the district honors parent and teacher requests. If during the school year there are academic concerns raised regarding your child, we may do a screening to rule out any vision or hearing problems.
Vision screening is not a substitute for a complete eye and vision evaluation by a doctor. Your child is not required to undergo this vision screening if an optometrist and/or ophthalmologist has completed and signed a report indicating that an examination has been administered within the previous twelve months.
